Sweeney Agency on Canisteo St in Hornell; at Brand Name Appliance, Main St. Wellsville; or at Club 57 in Hornell. One per visit but visit all 3 locations and register as many times as you want. Grand Prize is a 40 inch Samsung smart TV.
