FROM OAK ORCHARD MEDICAL: Effective August 1st, Rochester Regional Health’s Canisteo Valley Family Practice (CVFP) will become part of Oak Orchard Health—an established community healthcare provider with a presence in Hornell and ten locations across Western New York. Oak Orchard Health was chosen to continue the CVFP mission because of its strong local roots and shared commitment to patient-centered care.
Oak Orchard Health’s Hornell Health Center has been serving the area since 2015, offering primary care, pediatric services, dental care, and behavioral health services. Patients of Canisteo Valley Family Practice will now have access to those services.
CVFP is committed to ensuring that patients receive the best possible care moving forward. This transition ensures even more access to high-quality, compassionate care for patients right in the local community.

Starting August 1st, patients will be seen at the Oak Orchard Health Hornell Office at 7309 Seneca Road North, Suite 112, Hornell, NY. There will be no change in how you contact your provider. You can continue to reach them using the same phone number, (607) 324-5404, you use today, even after the transition.
