Vaccination Clinic For Allegany County On Feb 12th

News Release: Allegany County Department of Health COVID-19 Vaccination site for phase 1B essential workers only

The Allegany County Department of Health (ACDOH) has received guidance from the New York State that we will
be prioritizing essential workers in phase 1-B*. The ACDOH will hold a COVID-19 Vaccine Point of Distribution
(POD) clinic at the County Office Building located at 7 Court Street in Belmont NY 14813. The link to register will be
posted on the www.alleganyco.com website at 5pm on Wednesday, February 10, 2021. Vaccine is limited (200 doses)
so please be patient if you do not get in this round.

This clinic is only open for individuals that are essential workers in phase 1-B*. A limited number of
vaccinations will be available at this clinic (200 doses).

PLEASE NOTE: If you do not meet the criteria; do not have proof you meet the criteria in phase 1B; or
do not have an appointment you may be turned away at the clinic.

Step #1: Proof you are an essential worker in phase 1-B

To determine if you are an essential worker in phase1-B, go to the following link to determine your eligibility:
https://covid19vaccine.health.ny.gov/ . Click on “Am I eligible” and complete the form. Upon submission it
will advise if you are eligible or not. Please print out the page or show it at registration that tells you if you
are eligible and bring it with you to the clinic. You can also bring your work ID; a letter from your employer; or
a pay stub from your work. If you are a college instructor, you must be in person with students. THIS FORM
IS NOT A REGISTRATION FOR A VACCINE APPOINTMENT!

Step#2: How to register for a COVID-19 Vaccination
Vaccinations are available by appointment only. The link to register will be posted on the www.alleganyco.com
website at 5pm on Wednesday, February 10, 2021. Vaccine is limited so please be patient if you do not get in this
round. On the main page in the green box click the blue button “vaccine information” go to the LINK FOR COVID-19
VACCINE REGISTRATION: (the link will be here if there is a vaccination clinic.) Pick a time for your
appointment, enter your information and answer all the questions. Click the button at the bottom to register. Make
sure you complete your registration. You will get a confirmation email.

Step 3: Complete the Vaccine Form.
Once you have a confirmed appointment, you must complete the New York State COVID-19 Vaccine Form. Go
to https://forms.ny.gov/s3/vaccine. This form can be completed online and you will receive a submission ID, or
can be completed at your vaccination site. (We encourage New Yorkers to complete the form ahead of time.)

*Eligible New Yorkers in Phase 1-B essential workers are:
 First Responder and Support Staff for First Responder Agencies
o Fire Service (State and Local Fire Service, including firefighters and investigators (professional and volunteer)
o Police and Investigators (State Police, including Troopers; State Park Police, DEC Police, Forest Rangers; SUNY Police; Sheriffs’ Offices; County Police Departments and Police Districts; City, Town, and Village Police Departments; Transit of other Public Authority Police Departments; State Field Investigators, including Department of Motor Vehicles, State Commission of Correction, Justice Center, Department of Financial Services, Inspector General, Department of Tax and Finance, Office of Children and Family Services, and State Liquor Authority)
o Public Safety Communications (Emergency Communication and Public Safety Answering Point Personnel, including dispatchers and technicians)
o Other Sworn and Civilian Personnel (Court Officers; Other Police or Peace Officers; Support or Civilian Staff for Any of the above services, agencies, or facilities)
 Corrections (State Department of Corrections and Community Supervision Personnel, including correction and parole officers; Local Correctional Facilities, including correction officers; Local Probation Departments, including probation officers; State Juvenile Detention and Rehabilitation Facilities; Local Juvenile Detention and Rehabilitation Facilities)
 In-person college instructors
 P-12 Schools-faculty or staff (includes all teachers, substitute teachers, student teachers, school administrators, paraprofessional staff and support staff including bus drivers); Contractors working in a P-12 school or school district (including contracted bus drivers); Licensed, registered, approved or legally exempt group Childcare Providers; Exempt Childcare setting
 Grocery store workers
 Employees or Support Staff of Licensed or Registered Childcare Setting
 Licensed, Registered, Approved or Legally Exempt Childcare Providers
 Public Transit (Airline and airport employees; Passenger railroad employees; Subway and mass transit employees; Ferry employees; Port Authority employees; Public bus employees)
 Individuals living in a homeless shelter where sleeping, bathing or eating accommodations must be shared with individuals and families who are not part of the same household
 Individual working (paid or unpaid) in a homeless shelter where sleeping, bathing or eating accommodations must be shared by individuals and families who are not part of the same household, in a position where there is potential for interaction with shelter residents
 Newly eligible individuals in week 9, which include restaurant workers, restaurant delivery drivers, and for-hire vehicle drivers
